One filesystem access OUTSIDE the project root, awaiting the host's decision.

Passed to ent_path_approval_cb. Every string is engine-owned and valid only for the callback's duration — copy what must be retained. path is the CANONICAL path the tool resolved (symlinks and .. already resolved), so what the host shows the user is what would be touched, not what the model typed.
